Penile Torsion - Surgery and Traction
Penile Torsion is a rare and serious condition that causes the penis to become "wound". This affects the ability to become erect (normally prohibiting it) and can be painful to urinate. If penile torsion is from birth and not corrected, length and girth of the penis is normally stunted considerably. In recent studies, surgeons have attempted to cut the skin from the penis to "un-wind" it. However these surgeries have only been successful in around 30% of patients.
Traction devices are being used in studies to correct penis torsion, however no results have been released for these trials. However surgeons and urologists are optimistic that they can help straighten the penis.
Gene Therapy for Erectile Dysfunction
Arnold Melman, MD, Professor and Chair, Albert Einstein College of Medicine presented "Gene Therapy" for erectile dysfunction. In this fascinating discussion, Dr. Melman explained the rationale for this approach. He discussed the fact that K+ channels are ubiquitous and important in the contraction and vasodilatation that is so essential in erectile function. He pointed out the gene therapy for the penis did not require that all cells be treated - unlike therapy for cancer where all cells require treatment to eradicate the cancer. Using retrovirus with nuclear integration is thought to be safe in cell lines that are not replicating as opposed to those that are normally replicating (hematopoetic cell lines). Preliminary studies of eleven patients have been carried out establishing efficacy, safety and a dose response without adverse sequela after 2 years. This is an exciting and potentially meaningful avenue of research that is being pursued by private industry.

Prostate Cancer Screening at 40
Dr. Robert Nadler, of Northwestern University, wrote an excellent editorial in a recent issue of Cancer regarding the commencement of prostate cancer (CaP) screening at age 40. The National Comprehensive Cancer Care Network (NCCN) recommends an initial PSA at age 40 for men of all races. The frequency of subsequent PSA tests would depend on the initial level. Dr Nadler refers to work by Scales and associates that analyzed data from the 2002 Behavioral Risk Factor Surveillance System. This database is an annual, population-based survey from the Centers for Disease Control that determines the rate of screening in men between the ages of 40 and 50 years. It reports that 22.5% of men ages 40-49 had a PSA test the previous year, compared to 53.7% of men older than 50 years. More African-American men than Caucasian men were screened (33.6% vs. 21.5%). Nadler believes the data is encouraging in light of the fact that most primary care physicians are not aware of guidelines to screen men at age 40.
